Agencies may submit referrals for clients who may benefit from program-based independent living. 

1. Complete the Referral Form
2. Include relevant information (income, timeline, goals)
3. Await screening and availability confirmation

This form helps us determine program fit. Completing it does not guarantee placement. Housing applications may only be submitted after completion of the pre-eligibility process and confirmation from RAD Independent Living to proceed. 

Eligibility and placement are based on availability, program fit, and verification of information.
